Through interactive lectures, discussion of case studies, 8 self-assessment exams and exhibits, we present the most up-to-date information covering the principles of diabetes self-management education (DSME). If you are studying for the CDE examination, many people find it helpful to take the DCCW since we help you to identify what you know and what you still need to study to acquire proficiency.

 Purpose: To build capacity through partnership. 

 

Goal: Decrease and/or prevent tobacco use among those disparately affected by, or at risk for, diabetes, breast or cervical cancer, cardiovascular disease or stroke, and within specific populations (African American, Hispanic/Latinos, Asian/Pacific Islander, and LGBT). 

 

Impact: Elimination of tobacco related health disparities among specific groups.

BE A BRIDGE Capacity Survey Brief Summary of Findings

The BE A BRIDGE Partnership aims to use a collaborative approach to build capacity among a broad spectrum of organizations for the purpose of linking tobacco resources to chronic disease efforts in Pennsylvania.  Among the core BE A BRIDGE initiatives are three Regional Summits in Eastern, Central and Western Pennsylvania, each to be followed with skill-based workshops.  The BE A BRIDGE Capacity Survey was developed by Philadelphia Health Management Corporation (PHMC) to solicit feedback from potential Summit and follow-up workshop attendees that would inform the planning of these events as well as to assess current capacity and training needs of organizations within Pennsylvania.  The survey was distributed through BE A BRIDGE partner networks yielding 209 anonymous responses across Pennsylvania.
